Your feedback is very important to us! For each workshop or session you attend, please complete the evaluation form below or in the event mobile app. If the embedded evaluation form does not populate below, click here to access the Google Form directly.

While your email address is required to prevent duplication, your identity will remain confidential and will not be shared with the speakers or licensing boards. If you cannot find a workshop or session in the program list or prefer to submit your feedback anonymously, please use the paper evaluation form available at each workshop/session sign-in desk.
